Cancellation Policy:

Member may cancel their membership by providing a courtesy cancellation notice at least 10 days before the next charge. Cancellation notices must be submitted in writing to hello@formfitnh.com.

Expiration:

Sessions included in the membership do not roll over from month to month and will expire if not used within the month.

All packages must be used within 6 months of purchase, afterwards they will expire.

Missed Appointments:

For any late cancels (less than 12 hours before class time) or no-shows, the class will be taken from your account as if you used it, and you will be charged a $15 cancellation/no-show fee.
